При попытке создания запроса на первом шаге всё работает, а вот на втором: вписываем первый вариант и нажимаем "плюс", после чего должно появиться окно ввода второго варианта (как я понимаю), но, в итоге, отбрасывает на первый шаг с обнулением записей.
